Lakes & Prairies Community Action Partnership is proud to partner with the Department of Human Services - Office of Economic, other Minnesota Community Action Agencies, and local social service and community partners in this Food Support Awareness and Outreach campaign to increase participation in the Food Support Program.
The Food Support Program message is similar to other outreach messages. In most cases it will probably have to be heard 7 or more times before a lot of the people who may be eligible finally apply. So, share these resources with as many community partners as you can (at least 7 others)

Our goal in creating this site is to reduce or eliminate challenges of finding and printing resources as well as formats. You will find form numbers with a link to the MN e-docs website.
